Your first appointment

Upon your arrival at our clinic, our team might still need to ask you some questions if anything on your forms needs to be clarified. After that, you will meet with one of our dentists for a complete oral examination. This means we will be checking your teeth and the inside of your mouth. Whether you will need X-rays or not will be at the recommendation of your dentist.

After the examination, you will have a consultation period where your dentist will let you know about any issues they have found. Your dentist will discuss with you your options for treatment or procedures. This will be a great time for you to raise any questions you have, so don’t be afraid to ask them.

Patient Forms

Since we’re just getting to know you, you will need to fill out a patient form. This form includes a brief questionnaire about your medical and dental history. You will also need to fill out a release form. For that, you will need information about your previous dentist. This is so you can give them consent to forward your medical records to our clinic. For patients who prefer their insurance cheques to come to the clinic, a form is available for that as well.

You can download all these forms straight from this website, fill them out, then bring them with you on your appointment.

COVID-19 safety precautions

For your safety and ours, we will be asking you additional screening questions. On the day of your appointment, we ask that you wait in your car rather than our waiting room. We also encourage our patients to wear a face mask or covering when entering the clinic. Please be sure to sanitize your hands upon entering and leaving the office as well.

Our operations may be slightly different than before, but we are excited to welcome new patients safely. We thank you for your patience.

Thanks for letting Woo Dental take care of your smile. We’re happy to have you with us and we look forward to seeing you soon!
